Agreement to Participate
Parent/Guardian Confirmation And Signature
INDEMNIFICATION, GENERAL RELEASE & ASSUMPTION OF RISK
In consideration of my being allowed or my child for whom I am a parent or guardian ("Minor Child") being allowed to participate in any WEEKEND WARRIORS BASEBALL LLC activity or program, including but not limited to baseball, softball, soccer, lacrosse, strength & agility training, and camps, I agree to forever release, indemnify, hold harmless and discharge WEEKEND WARRIORS BASEBALL LLC, their members, affiliates, directors, officers, employees, agents, sponsors and independent contractors (former and present), individually and in their official capacities (former and present), on behalf of myself, my spouse, my children, the Minor Child, my parents, my guardians, and my heirs, assigns, personal representative and estate, and any and all other persons and entities who could in any way represent me, the Minor Child or act on our respective behalves from any and all actions, causes of action, suits, charges, complaints, proceedings, grievances, obligations, costs, losses, damages, injuries, attorneys' fees, debts, dues, sum of money, accounts, covenants, contracts, controversies, agreements and promises of any form whatsoever ("claims") including, but not limited to, any claims in law, equity, contract, tort, all federal, state, county or local laws, or those claims which were or could have been alleged with respect to participation, or otherwise arising as a result of participation.
I acknowledge that participation by myself and/or the Minor Child entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death or damage to persons or property, including illness from diseases such as MRSA, influenza, and COVID-19. I voluntarily assume all risks and choose to participate or permit participation despite the risks.
If I or the Minor Child is injured or becomes ill, we may require emergency/medical assistance, at our own expense.
I certify that I have adequate insurance to cover any injury, illness, or damage I or the Minor Child may cause or suffer while participating, or else I agree to bear the costs of such injury, illness or damage myself. I further certify that I and the Minor Child are willing to assume the risk of any medical or physical condition we may have.
I agree that this release is also made on behalf of the Minor Child. I represent that I have full authority as parent or legal guardian to bind the Minor Child to this agreement, and I further agree to defend and indemnify WEEKEND WARRIORS BASEBALL LLC from any claims brought by or on behalf of the Minor Child.
I do hereby authorize WEEKEND WARRIORS BASEBALL LLC and their assigns to utilize any and all photographs, pictures or other likenesses of me or anyone under my guardianship in promotional materials or team films as deemed appropriate.
I have read and understood this agreement and had the opportunity to consult legal counsel. I voluntarily agree to be bound by its terms.
